Working Partners
 
The Community Health Foundation of Western & Central New York- is dedicated to improving the health and health care of the people and communities of western and central New York. 
 
State University of New York- Institute of Technology, School of Nursing and Health Systems (SUNYIT) -  Partners with the Foundation to provide a range of on-going support services  related to the Long Term Care Leadership Institute, such as ongoing participation of SUNY staff on the Leadership Institute Advisory Committee assistance in the development and refinement of curriculum for nursing education, review and approval of course content by the Curriculum Development Committee, general academic support including guest speakers, the provision of college credits to participants, and use of campus learning facilities and resources such as libraries, computer labs and career counseling centers.
 
New York State Department of Health (NYSDOH), and The Cornell Institute for Translational Research on Aging (CITRA) located at Cornell University - have partnered with the Foundation in an array of educational and research program areas, including the NYSDOH Home Health Aide/Personal Care Aide/ Certified Nurses Aide Core Curriculum Project; the Feeding Assistant Curriculum Project, the NYSDOH Dementia Grant Program; and the Study of End-Of-Life Practices in New York State Nursing Homes.
 
New York State Nurses Association (NYSNA)Partners with the Foundation to provide contact hours and continuing education units to participants in the Nurse Leadership and Nurse Educators Course. NYSNA also has a representative that participates on the Leadership Institute Advisory Committee.
 
Funding Partners
  
New York State Department of Health (NYSDOH) Between 2007-2010, NYSDOH provided the Foundation with $802,000 of support from the Health Workforce Retraining Initiative (HWRI) to fund the Long Term Care Leadership Institute.
 
Community Health Foundation of Western and Central New York - Between 2008-2011, CHFWCNY will provide FQC with $65,196 of support to fund the Improving Care and Caring Program, which is a quality improvement training series for long term care professionals.
 

U.S. Health Resources and Services Administration (HRSA) Between 2007-2010, HRSA provided the Foundation with $415,000 of support to fund in part the Long Term Care Leadership Institute.

 
Jacob and Valeria Langeloth Foundation – Between 2008-2011, Langeloth Foundation will provide FQC with $475,000 to fund the Geriatric Nursing Assistant Career Development Program, which is a career ladder education and recruitment program for certified nursing assistant (CNAs) working in nursing homes.
