This program has two tracks. Track One: is designed for participants from Skilled Nursing Facilities and includes all six sessions. Track Two: is designed for participants from Adult Care Facilities, Assisted Living, or Home Care and includes four sessions (1, 4, 5, 6). All sessions will be held on Thursdays.
